Where are we located?
Outside the North Tulsa Costco on the right of the main entrance, not inside the warehouse itself. Costco Memberships are not required to come to our office!
What you need to know before scheduling an appointment
If you have vision coverage we can file for out of network reimbursement on your behalf. Many participating vision insurance plans such as VSP, EyeMed, and Davis Vision will qualify. Your insurance then mails you a check for your visit in 1-2 weeks free of any additional charge.
In simple terms, you will pay full price for your visit initially, if we file for reimbursement for you at the end of your visit, the money covered by your insurance is mailed to you.
We are out of network for all insurance (unless you are a Costco employee).
